This section is for those who have decided to enroll for their own personal account. Please read the following to fully understand the process of registration and the cost of enrollment.

The MYFITNESSLIFE is an annual subscription of twelve consecutive calendar months. By registering at any time in one calendar month you get the remainder of that initial month for free. (This method keeps all subscriptions on an even calendar month basis starting on the 1^st day of the first month of the subscription just like a magazine subscription.)

The system is offered in two parts. Module-A refers to the basic Medical Information module and the extended system including Module-B, the Monitoring module. We offer Option-A and Option-AB. The basic module is A for Medical-only and AB is for the extended system linking together Module-A and Module-B.

Option-A alone is suitable for the individual who is interested in maintaining only a medical file for emergency access or a medical reference for a Caregiver. Option-AB is for those individuals who want to use the full-featured system. The "My Dashboard" menu page shows the features comprising the A-module (left-side, light blue). The B-module components are listed on the right-side in blue. All non-blue functions are common to both Option-A and Option-AB.

Option-A is $8.99 for a 12-month subscription or less than $0.75 per month.

The Standard Option-AB is $11.88 for a 12 month subscription or a $0.99 per month rate.

The function modules of either A or B are displayed on the left and right sides of the "My Dashboard". There are no shipping or handling fees or any other processing fees.

Payment for a subscription is by credit card only. MYFITNESSLIFE accepts either VISA or Mastercard. MYFITNESSLIFE pays all transaction and credit card fees and associated charges. If you are not entirely satisfied that the subscription is appropriate for your needs, we request that you do not to enroll until you are certain of its value. (The subscription’s low cost makes the overhead of cancellation prohibitive and therefore not feasible for pro-rated reimbursement.)

Annual Renewal is not automatic for the next year because the credit card payment data is not retained by the system as part of our privacy policy. MYFITNESSLIFE will notify you 30 days in advance of expiration by email so that you can renew again in time to avoid any interruption and the one-time origination fee. You should be diligent to keep your email address current if it changes anytime in the future.

Step #1 – Acceptance of the MYFITNESSLIFE Service Agreement.

Step #2 – Credit Card Payment submittal and confirmation using the Individual or Family Plan.

Step #3 – Commencement of Subscriber account activation.
